UnitedHealthcare/Optum
Participation may vary by clinician, specific plan, employer group, service type and the state where the client is physically located at the time of service. Being listed here does not mean every product offered by an insurance company is accepted.
Insurance Verification
We will attempt to verify your eligibility and benefits before services whenever possible.
Verification is an estimate based on information supplied by your insurance company and is not a guarantee of coverage or payment.
Your insurance company makes the final decision regarding:
-
Eligibility
-
In-network or out-of-network status
-
Medical necessity
-
Covered and excluded services
-
Copayments, coinsurance and deductibles
-
Authorization requirements
-
Claim processing and payment
-
Your final financial responsibility
We encourage you to contact the member-services number on your insurance card to confirm behavioral-health and telehealth benefits.
02 - CLIENT FINANCIAL RESPONSIBILITY
What Insurance May Leave You to Pay
Common Patient Responsibility
Clients are responsible for amounts assigned to them under their insurance plan or payment agreement, including applicable:
-
Copayments
-
Coinsurance
-
Deductibles
-
Self-pay fees
-
Non-covered or excluded services
-
Late-cancellation or missed-appointment fees
-
Balances resulting from denied or adjusted claims
-
Services received after coverage ends or changes
Keep Us Informed
Clients are responsible for providing accurate insurance and subscriber information before services and notifying Milton’s Place promptly about changes in coverage, employment, address, coordination of benefits or primary insurance.
If an insurance claim is denied because information was inaccurate, incomplete or outdated, the resulting balance may become the client’s responsibility unless prohibited by law or an applicable insurance contract.
Milton’s Place will assist with reasonable claim questions, but an insurance policy remains an agreement between the client and the insurance company.
03 - PAYMENT METHODS AND CARD ON FILE
Predictable Billing Process
Accepted Payment Methods
Milton’s Place accepts major credit and debit cards and eligible HSA/FSA cards through its secure payment system.
Clients may be required to maintain an authorized payment method on file. The secure payment processor manages payment information. Milton’s Place does not directly store full card numbers.
Card-on-File Authorization
A signed or electronic authorization allows the approved payment method to be used for applicable:
-
Self-pay session fees
-
Copayments, coinsurance and deductibles
-
Patient responsibility determined after claim processing
-
Late-cancellation or missed-appointment fees
-
Previously discussed or approved balances
Charges will be processed only in accordance with the signed authorization and applicable client policies.
Billing Timing
Known copayments, self-pay fees and other established charges may be processed at the time of service.
When insurance is billed, the remaining client responsibility is generally processed after the insurer adjudicates the claim. This may occur several weeks after the appointment.
Receipts and account documentation are available through the secure client portal or upon request.
04 - HSA AND FSA PAYMENTS
Using YourHSA/FSA
Milton’s Place can provide itemized receipts or eligible super-bills upon request but cannot determine whether a particular expense qualifies for reimbursement.
Eligible HSA or FSA cards may be used for qualifying counseling services.
Clients are responsible for confirming that a service and expense qualify under their individual plan and for maintaining any documentation required by the plan administrator or applicable tax rules.
05 - SELF-PAY, OUT-OF-NETWORK SERVICES, AND GOOD FAITH ESTIMATES
Expected Cost of Care
Self-pay and Out of Network Services
Clients who do not use insurance will receive information about the applicable self-pay rate before services begin.
Clients with out-of-network benefits may request a super-bill containing information commonly required for reimbursement, including:
-
Client and provider information
-
Diagnosis and procedure codes
-
Dates of service
-
Place of service
-
Amount charged and paid status
Super-bills are generally delivered through the secure client portal. Providing a superbill does not guarantee that an insurance company will reimburse any portion of the service.
Clients are responsible for contacting their insurer to determine whether out-of-network benefits are available and what documentation is required.
Good Faith Estimate Rights
Clients who are uninsured or choose not to use insurance have the right to receive a Good Faith Estimate explaining the expected cost of scheduled services, as required by federal law.
A Good Faith Estimate may be provided when services are scheduled or upon request. The estimate is based on information known at the time and is not a guarantee of the exact number, frequency or type of future services.
If treatment needs, service frequency or other circumstances change, the expected cost may also change. Updated information will be provided when appropriate.
If a final bill from a provider is at least $400 more than the expected charges listed in that provider’s Good Faith Estimate, the client may be eligible to use the federal patient-provider dispute-resolution process.
06 - FINANCIAL DOCUMENTS AND LIMITED OPTIONS
Receipts, Super-bills, and Financial-hardship Requests
Itemized Receipts & Super-bills
Itemized receipts and eligible superbills may be requested for self-pay, out-of-network, HSA/FSA or other reimbursement purposes.
Requests will generally be completed through the secure client portal. Processing time may vary depending on the requested dates and documentation.
Because super-bills typically include diagnosis and procedure information, clients should consider the privacy implications before submitting one to an insurance company or other third party.
Financial Hardship & Sliding Scale
A limited number of reduced-fee openings may be available for clients experiencing financial hardship.
Requests must be discussed directly with Milton’s Place and approved before the reduced rate is used. Availability is limited and is not guaranteed.
Factors that may be considered include:
-
Job loss
-
Significant reduction in income
-
Major medical expenses
-
Loss or change of insurance coverage
-
Other substantial financial hardship
Milton's Place may request documentation when appropriate.
Reduced-fee arrangements may be reviewed periodically and may change based on availability or financial circumstances.
07 - REFUNDS, CREDITS AND BILLING QUESTIONS
Clear Expecations
Late Cancellations and No-Shows
Applicable fees for cancellation notice periods and no-shows are listed in the signed cancellation policy and are generally not covered by insurance.
Repeated cancellations and no-shows are discussed in client-relationship documentation signed during the intake process.
Refunds and Credits
Confirmed overpayments are generally refunded to the original payment method within 14 business days when possible. Bank or card-processing timelines may affect when the refund appears.
With written client consent, an account credit may be applied to a future service instead of being refunded.
Billing Questions
If you believe a charge, insurance claim or account balance is incorrect, please contact us promptly. We can review available account information, insurer explanations of benefits and itemized documentation.
Contacting Milton’s Place about a billing concern does not limit any rights available under applicable law or an insurance contract.
Milton’s Place may update this page to reflect changes in payment methods, insurance participation, billing practices or applicable requirements.
Updates will be posted here with a revised “Last updated” date. Changes to this website page do not replace or retroactively modify signed client agreements unless a revised agreement is provided or applicable law requires otherwise.
