Acceptance of Terms
Agreement to Terms
By accessing or using Milton's Place services, including our website and telehealth platform, you agree to be bound by these Terms of Service and all applicable laws and regulations.
Capacity to Contract
You represent that you are at least 18 years old and have the legal capacity to enter into this agreement. If you are under 18, a parent or guardian must agree to these terms on your behalf.
Modifications
We reserve the right to modify these terms at any time. We will notify you of material changes, and your continued use of our services constitutes acceptance of the updated terms.
Description of Services
Mental Health Services
Milton's Place provides professional mental health counseling services via secure telehealth technology to clients located in Michigan and Minnesota. Our services include individual therapy, couples counseling, and group therapy sessions.
Licensed Practice
All services are provided by a licensed professional counselor in accordance with Michigan state laws (for Michigan clients), Minnesota state laws (for Minnesota clients), and professional ethical standards.
Telehealth Platform
Our services are delivered through HIPAA-compliant telehealth technology. You are responsible for ensuring you have appropriate technology and internet connectivity for sessions.
Client Responsibilities
Accurate Information
You agree to provide accurate, complete, and current information about yourself and your health status. You must promptly update any changes to your information.
Session Participation
You agree to participate in sessions from a private, secure location and to maintain confidentiality of other group members if participating in group therapy.
Technology Requirements
You are responsible for ensuring you have reliable internet, appropriate devices, and a private space for telehealth sessions. Technical issues on your end may result in session rescheduling.
Scheduling and Cancellation
Appointment Scheduling
Appointments must be scheduled in advance through our approved scheduling system or by contacting our office directly during business hours.
Cancellation Policy
You must provide at least 24 hours notice to cancel or reschedule an appointment. Late cancellations or no-shows may result in charges as outlined in our fee schedule.
Provider Cancellations
If we need to cancel or reschedule your appointment, we will provide as much notice as possible and work with you to find an alternative time.
Payment and Fees
Fee Structure
Our current fee schedule is available upon request and may vary based on the type of service provided. Fees are subject to change with 30 days written notice.
Insurance and Payment
We accept various insurance plans and also offer self-pay options. You are responsible for understanding your insurance benefits and any applicable copays, deductibles, or out-of-network costs.
Late Fees and Collections
Unpaid balances may be subject to late fees. Accounts significantly past due may be referred to collections or result in suspension of services.
Confidentiality and Privacy
Therapeutic Relationship
Information shared during therapy sessions is confidential and protected by law, except in specific circumstances outlined in our Privacy Policy and as required by law.
Limits of Confidentiality
Confidentiality may be limited in cases involving risk of harm to self or others, suspected abuse, court orders, or other legal requirements as outlined in our Privacy Policy.
Technology and Security
While we use secure, HIPAA-compliant technology, no electronic communication is 100% secure. You acknowledge and accept the inherent risks of telehealth communication.

Termination of Services
Client-Initiated Termination
You may terminate services at any time by providing written notice. We recommend discussing termination with your therapist to ensure appropriate closure and transition planning.
Provider-Initiated Termination
We may terminate services for reasons including non-payment, violation of these terms, inappropriate behavior, or when services are no longer clinically appropriate.
Records and Referrals
Upon termination, we will provide appropriate referrals when clinically indicated and will maintain your records according to legal and professional requirements.
Liability and Governing Law
Limitation of Liability
To the fullest extent permitted by law, Milton's Place, PLLC and its providers shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services, except as required by professional liability standards.
Governing Law
- Michigan Clients: These terms are governed by the laws of the State of Michigan. Any disputes arising from these terms or our services shall be resolved in the appropriate courts of Michigan.
- Minnesota Clients: These terms are governed by the laws of the State of Minnesota. Any disputes arising from these terms or our services shall be resolved in the appropriate courts of Minnesota.
Professional Standards
- Michigan Clients: Our services are provided in accordance with applicable professional ethical standards, including those of the American Counseling Association and Michigan licensing requirements.
