Payments Policy

This policy outlines payment methods and rules for Milton’s Place, PLLC.

Insurance & Payment Information

We work with most major insurance providers to make mental health care accessible.

We also offer sliding scale fees for those without insurance or with high deductibles.

Card On File

  • Privacy: We do not store full card numbers locally.
     

  • Authorization: We require an authorization/consent form to keep a card on file for session fees, late/cancel fees, and approved balances.
     

  • Billing Timing: Your card will be charged at time of service or upon claim adjudication for patient responsibility.
     

  • Disputed Charges: We will provide itemized receipts and a Good‑Faith Estimate when requested.

HSA/FSA Acceptance

  • We accept HSA/FSA cards for eligible services.
     

  • Clients are responsible for confirming eligibility.
     

  • We can provide itemized receipts or superbills upon request.

Superbill Generation

  • For out‑of‑network/self‑pay, we can generate superbills with client info, provider info (NPI/EIN), diagnosis (ICD‑10‑CM), CPT, POS, dates, units, amount, and paid status.
     

  • Superbill can be sent via client portal or encrypted email.
     

  • Sent per request.

Sliding Scale & Financial Hardship

  • Sliding scale fee adjustments may be available on a limited basis for clients experiencing financial hardship.
     

  • Requests must be discussed directly with the clinician prior to approval.
     

  • Availability of reduced fees is limited and not guaranteed.
     

  • Financial hardship may include job loss, medical expenses, reduced income, or other significant financial strain.
     

  • Documentation may be requested to support hardship consideration when appropriate.

Refunds & Credits

  • Overpayments are refunded to original payment method within 14 business days when possible.
     

  • Credits may be applied to next service upon client written consent.

Changes to This Payments Policy

We may update this payments policy from time to time to reflect changes in our practices or applicable laws. We will notify you of any material changes by posting the updated policy on our website and updating the "last updated" date.

Your continued use of our services after any changes indicates your acceptance of the updated payments policy.
